Henoch-Schönlein Purpura (Henoch-Schonlein Purpura)

OVERVIEW

Henoch-Schönlein purpura (Henoch-Schonlein purpurn) is an acute, systemic small vessel vasculitis.

WHAT CAUSES IT?

This is an IgA-mediated vasculaits.

WHAT MAKES US SUSPECT IT?

Initial Presentation

  • Rash: palpable purpura 
  • Arthritis/arthralgia 
  • Abdominal pain
  • Hematuria 
CLINICAL WORKUP

Serum studies:

  • Normal platelets
  • Normal coagulation studies
  • Normal/elevated creatinine
  • Hematuria often present (proteinuria may also be present but may be absent)
HOW DO WE TREAT IT?

Supportive measures:

  • Hydration
  • NSAIDS

Patients with severe symptoms:

  • Hospitalization
  • Systemic glucocorticoids
